Engagement Trip Forms

There are several things our office needs to have on file for you before you leave for Indonesia! Please see the list below.

1) PASSPORT PHOTOCOPY

Due: ASAP to ensure your spot on the team, no later than 3 months before the departure date.
Once you are accepted onto a trip, you will be asked to send in a photocopy of your passport to hold your spot on the team.  If you do not have a passport yet, you will need to confirm that you have ordered your passport and send in your name and date of birth as they will appear on your passport. This information will be used to purchase your flights.

Your passport’s expiration date must not be within 6 months of your departure date. If this is the case, you will need to renew your passport. 

 

2) BACKGROUND CHECK

Due: ASAP to ensure your spot on the team, no later than 3 months before the departure date.
Along with submitting your passport, you will also need to undergo a background check. You will receive an email with instructions on how to complete a background check online.

 

3) ENGAGEMENT TEAM PACKET: Click Here to Download

Due: 2 weeks before the departure date.
Read, sign, and submit an Engagement Team Packet.
Your team leader will go over what is covered in this packet with you.

 

4) MEDICAL FORM: Click Here to Download

Due: 2 weeks before the departure date.
Get the suggested vaccinations and submit a Medical Form.
We provide Trip Insurance for all team members. See details on the Premium Plan offered by Faith Ventures here.

 

5) TRIP PAYMENT 

Due: 2 weeks before the departure date.
Currently, we charge $3,000 for a trip to Indonesia. This covers your flights, lodging, and meals. You can pay for your trip all at once, in installments, or have friends and family help you fundraise. We ask that any large trip payments be made via check. Please instruct your friends and family to write your name in the memo, if they are sending in a check. All donations to your trip payment are tax-deductible, and any amount raised above the $3,000 will be received as general donations to Compassion First. If you cancel your trip after your flight has been booked, you are responsible for the $300 cancelation fee.
